Skin Grafts Care of Donor Sites

 Prepare all necessary equipment and supplies.

   1. Wash hands well, and don examination gloves as needed.
   2. Remove gauze roll and any padding covering inner dressing.
   3. Assess the donor site for signs of healing and complications; assess whether the inner dressing needs to be        changed. Proceed to step 8 if inner donor dressing needs to be changed.
   4. Remove and discard gloves, and don a pair of clean gloves.
   5. Gently wash exudate from wound edges with warm tap water, and pat dry.
   6. Use scissors to trim loose edges of donor site dressing. If inner dressing does not need to be changed,               proceed to step 14 to complete donor site care.

Inner Dressing Change

   1. Remove inner dressing and discard it.
   2. Remove and discard gloves, and don a pair of clean gloves.
   3. Gently wash wound with mild solution, rinse with warm tap water, and pat dry.
   4. Assess the donor site for progression of healing and complications; outline any inflammation with a marking           pen.
   5. Remove and discard gloves. Don a pair of sterile or clean gloves.
   6. Cut dressing to the size of donor site with sterile scissors, apply, and secure in place.
   7. Remove and discard gloves. Wash hands.
   8. Reapply bulky outer dressing if indicated.
   9. If patient has a transparent film dressing, change according to physician orders only.  Observe for progression        of healing and complications.
  10. Document in patient record.
