Vaginal Exam (VE)
Perform hand hygiene and apply clean gloves.
Performing a Sterile Vaginal Exam
  1. Vaginal Exam (VE)
  2. Perform hand hygiene and apply clean gloves.
  3. Verify the patient's identity using two identifiers in accordance with institutional policy.
  4. Provide pericare. Remove gloves and discard in proper trash receptacle. Perform hand hygiene.
  5. Apply sterile glove to dominant hand used for performing VE.
  6. Apply lubricant to second and third fingers of gloved hand.
  7. Encourage the patient to breathe slowly and deeply with insertion of second and third fingers into the vagina.
  8. Assess dilation, effacement, and position of the cervix, presenting part, position and station of fetus, and status of membranes.
  9. Remove glove and discard in proper trash receptacle. Perform hand hygiene and apply clean gloves.
  10. Provide pericare.
  11. Remove gloves and discard in proper trash receptacle. Perform hand hygiene.
  12. Report findings to the practitioner as appropriate.
  13. Document the procedure in the patient's record.
Assisting with Gynecologic VE
 
  1. Perform hand hygiene and apply clean gloves.
  2. Verify the patient's identity using two identifiers in accordance with institutional policy.
  3. Lubricate and warm the speculum in warm water.
  4. Instruct the patient to bear down as the speculum is inserted.
  5. Assist the practitioner with collection of specimens.
  6. Label specimens according to institutional policy.
  7. Remove gloves and discard in proper trash receptacle. Perform hand hygiene.
  8. Apply lubricant to the practitioner's second and third fingers of gloved hand.
  9. Assist the practitioner and support the patient as needed during bimanual examination.
  10. Apply clean gloves and provide pericare to the patient after examination is completed.
  11. Remove gloves and discard with supplies in proper trash receptacles. Perform hand hygiene.
  12. Assist the patient to sitting position and then to standing position if needed.
  13. Document the procedure in the patient's record. erify the patient's identity using two identifiers in accordance with institutional policy.
