Eye Prophylaxis Newborn

Assess the infant's eyes for any redness, edema, or drainage.

  1. Cleanse the eye area with gauze and sterile saline if there is any drainage or crusting, starting at the inner canthus. Perform hand hygiene before and after cleansing if cleansing is necessary. Obtain culture and sensitivity as necessary.
  2. Verify the chart for proper dosage and to ensure that eye prophylaxis was not already administered.
  3. Identify and obtain the medication.
  4. Double check the medication with a second nurse or as per institutional policy.
  5. Perform hand hygiene and apply clean gloves.
  6. Verify the patient's identity using two patient identifiers in accordance with institutional policy.
  7. Place gauze in the non-dominant hand and apply on the infant's cheekbone below the lower eyelid. Gently press downward to expose the lower conjunctival sac.
  8. Instill eye prophylaxis.
  9. Close the eye gently with gauze.
  10. Remove gloves and discard with supplies in proper trash receptacles. Perform hand hygiene.
  11. Continue to assess the infant for any local and/or systemic reactions.
  12. Document the procedure in the patient's record.
