Head Circumference Measurement

  1. Perform hand hygiene.
  2. Verify the patient's identity using two patient identifiers in accordance with institutional policy.
  3. Position the infant supine.
  4. Place a disposable flexible tape measure around the occipitofrontal circumference of the infant's head, positioning the tape measure above the infant's ears and just above the infant's eyebrows. Adjust the tape measure at the occipital bone to obtain the largest measurement and read the tape at the zero mark.
  5. Obtain a second measurement if the first was abnormal for the infant's gestational age or overall size.
  6. Remove the tape measure and discard it if it is contaminated, or cleanse it per institutional policy.
  7. Perform hand hygiene.
  8. Document the procedure in the patient's record.
